Cloud vs. On-Premise EMR: Which Is Right for You?

One of the first architectural decisions in any EMR purchase is where the software runs: in the vendor's cloud or on servers you own and maintain. The choice affects cost structure, IT workload, security responsibilities, and how the system behaves when your internet goes down. Neither model is universally better, the right answer depends on your resources and priorities.

How the two models differ

A cloud-based (SaaS) EMR is hosted by the vendor and accessed over the internet. The vendor manages servers, storage, backups, security patches, and software updates. You typically pay a recurring subscription.

An on-premise EMR runs on servers physically located at your practice. You own the hardware and are responsible for maintenance, backups, security, and updates, usually with help from IT staff or a contractor. Costs skew toward larger upfront investment.

Industry trend: The market has shifted strongly toward cloud-based delivery over the past decade, especially among small and mid-sized practices that don't want to run their own server infrastructure.

Security responsibilities

Both models can be HIPAA-compliant, but the responsibilities differ. With cloud hosting, you must have a business associate agreement with the vendor and rely on them for much of the technical security, though you remain responsible for access controls and workforce practices. With on-premise, more of the security burden, physical security, encryption, backups, falls directly on you.

  • Do you have reliable internet and a downtime plan? Cloud requires connectivity.
  • Do you have IT staff or a contractor who can maintain servers? If not, cloud reduces burden.
  • How do you prefer to budget, predictable subscriptions or larger capital purchases?
  • Do you need deep customization or control that an on-premise system might allow?

Plan for downtime either way

Both models can go down, and both need a downtime plan. With cloud, your risk is internet connectivity, so consider a backup connection and a paper or read-only fallback for when you're offline. With on-premise, your risk is local hardware failure, which makes tested backups and recovery procedures essential. Ask each vendor what happens during an outage: can clinicians still view recent records, and how quickly does service or access return? A system's behavior during a bad day matters as much as its features on a good one.

When on-premise still makes sense

Some larger organizations with mature IT departments, specific control requirements, or connectivity concerns still choose on-premise. Organizations in areas with unreliable internet, or those with strict internal policies about where data physically resides, may reasonably prefer to keep servers on site. But for most small and mid-sized practices, cloud delivery removes a significant operational burden and offers more predictable costs, which is why the market has moved decisively in that direction.
